HOW TO SUBMIT A FORM: 1. Open the Form Click on the form link. The PDF will open in a new window or tab. 2. How Do You Want to Complete It?  A. Digital Option: Complete the Form on Your Computer Click in the fillable fields and type your information directly into the form (Proceed to Step 4). B. Print Option: Print the Form by clicking the Print icon or pressing Ctrl and P on your keyboard. Fill It out by hand. 4. Submit the Form Choose a submission type listed on the form (ie: email it by scan and upload to your computer, mail it, or submit it in person) and follow the instructions.
